I'm a pilot and the FAA wants me to get an MWT-40 in order to renew my FAA medical certificate. They also want me to have a drug screening before the test. This is not surprising, but they also require that I be tested for caffeine. I work outside the USA, and the sleep study centers I have dealt with so far do not have a caffeine test available - they just ask if you drink coffee and leave it at that.

So I need an MWT-40 test somewhere that can also test for caffeine. Also, I don't have American health insurance, so I will be paying out of pocket. I am already diagnosed with OSA, have my pressures titrated, and have an AHI averaging about 4.0 and AI close to zero using my CPAP machine. I really don't want or need another PSG, and would rather not pay a few thousand dollars for a PSG I don't need. But it seems many clinics will not offer an MWT without giving a PSG.

Does anyone know a place I can get a MWT and a caffeine test without getting another PSG first? I have records from 2 PSG tests in the last year.

5.The use of tobacco, caffeine, and other medications by the patient before and during MWT should be addressed and decided upon by the sleep clinician before MWT. Drug screening may be indicated to ensure that sleepiness/wakefulness on the MWT is not influenced by substances other than medically prescribed drugs. Drug screening is usually performed on the morning of the MWT, but its timing and the circumstances of the testing may be modified by the clinician. A light breakfast is recommended at least 1 hour prior to the first trial, and a light lunch is recommended immediately after the termination of the secondnoon trial.

the people who want these test should provide names and addresses of places that carry out these tests to see if they are approved by them

the sleep clinian should be able to direct you to the caffeine approved test clinic

do sleep clinians have these addresses normally

How long ago was your last titration? If it has been within a year it is possible that it won't be required. You will have to provide proper documentation of the study. If your study was outside of a year you will more than likely have to go through the entire process again. Sorry, but I as someone who flies often I believe this to be necessary.
My last titration was 9 days ago. I'm hoping to find someone with personal knowledge of a moderately priced practitioner who is willing to give me the only test required for my medical renewal by the FAA, an MWT-40. This test is not about my treatment or health. It is about fulfilling paperwork to satisfy a bureaucracy. I don't need a caffeine test to tell me if I have caffeine in me, but the FAA does.
Erik, Do you know you are posting in an international forum to find a practitioner and not giving us a clue as to the physical locations you would be able to visit?

This test is not about your health. If you have been titrated then your health needs should have been addressed. This is about the health and safety of your passengers and co-workers. I would hope that their safety is important enough for you to have this procedure done.

If you have the documnents from your titration you will not need to have a repeat PSG. This site will help you find a testing center near you. Most labs offer a self pay discount.

Yes, I understand it is international. So far I have had a PSG in Thailand, the Philippines and Saudi Arabia. I don't really want to do a MWT in North Korea or Antarctica but just about anywhere else might be a possibility =). So far, the three places I have tested did not offer a caffeine test, and seemed mystified when I asked about it. So that might limit me to the USA or Europe. Most likely, the FAA would look with the most favor upon a test in the USA. I have tried talking to the FAA about what they will accept, but I could not get past the secretary to the actual aviation medical administrator. The secretary seemed to think that the procedure is identical everywhere in the world, but that is not the case. The places I called that are in areas close to my family in the USA quoted me $4000 for the PSG and $2000 more for the MWT. I don't have health insurance in the USA because I fly air ambulance for a foreign country. I don't really want to pay $6000 for what I got for under $1000 in other countries.
Rock, did you even read my posts? I had 2 PSG in the last year. I had a PSG and a MWT 9 days ago. I have a current aviation medical certificate issued by a country other than the USA. My MWT satisfies every aviation authority in the world except the USA, because the FAA wants a caffeine test. No other country requires this. I don't fly for an airline, and I don't fly in the USA. I just want to get my FAA medical in case I should ever need it. You talk like I am some cowboy trying to game the system, and I don't think I have said anything that would justify this..
No where in any of your post does it say that you had gone through an MWT. I apologize for interpretting your post in a certain way. that is not fair to you. Good luck! We still do not know what area you are looking for.
What I need is a sleep doctor who is:
1. Good
2. Not too expensive for someone who will be paying cash.
3. Independent enough to make a judgment on whether to give me an MWT-40 test without insisting on a PSG that I don't need, since I had one less than 2 weeks ago. Corporate clinics associated with HMO's don't seem willing to do this...
4. Associated with a lab that can test me for caffeine.
5. Located in the USA or Europe.

I have had PSGs in Asia and the Middle East, and they seemed to be professionally done, but when I asked about a caffeine test they said they don't do those because if they want to know if someone used something with caffeine in it, they just ask them. Yes, I'm aware of pilot forums, thx, but I thought someone here might have a suggestion for a sleep specialist they like, or perhaps even be one themselves.
I gave you the information that you needed in the above post. Do you have the documentation from your PSG and titration? If not you WILL have to repeat the entire process.
