I have read many posts on this site....and must admit I am confused as to why there is no support for using the auto-set cpap versus straight cpap??

I have done both sleep tests with and w/o my cpap machine.....I have a data capable machine....at first I was put on auto-set, settling at 4.0 up to 12 over 45 mins....it took some while but, I adjusted to it...then I went in for my second sleep test with my machine....it was decided to put me on straight cpap up 10 with a chin strap....so again--it took me a while to readjust but, I did......then I had a follow up apt with my sleep doc with all my data on auto-set and straight cpap-- it was determined that I did much better on auto-set with less AHI-8.5 and on straight cpap at 15.9 so here I am once again in the transition stage of getting adjusted to auto-set.

I am confused why there is so much dislike here for the auto-set when some of us fair better on this as per recommended by my sleep doctor....I am struggling with the third adjustment but, I am not giving up (the settling on auto-set is 6.0 to 15) could someone let me know if this is a good setting this was just changed as per my supplier....could I be struggling with it because 6.0 is to high???

I would greatly appreciate any feedback on this!!!

6 maybe too low. If they have the range set that far apart it maybe that you are having apneas while the machine raises the pressure. Mine is set to a 9 - 15 and that is considered to wide a range by many.

What is your average pressure?
I'm from Canada and I'm not able to set my own settings my supplier is the only one who has authority to do this....would I do better if I up my level higher??? Can anyone tell me how to do that myself it really is a pain to bring it into the city every time I need my levels increased....

Would I adjust better if my level were to be increased to 8???? All I want is to be on my machine each night every night....I still feel like I am choking not sure if this adjustment stuff or if my level needs to be increased??? I am just tired of being tired or is this normal to have this adjustment period like this???

Feedback would be appreciated
Hi Belinda,
I have an ASV, for @ 35 days now, with settings 4/12, when I first started using it, I felt like I wasn't getting enough air, and after some trial and error, I found I needed to use larger nasal pillows, Ithe smaller ones just didn't let the air flow, and they were collapsing on me too. I also use BreatheRight strips to hold my nostrils open, it seals a lot better that way, less leaks. Not sure if that is what your problem is, though..
Personally I believe that if you are titrated right that a straight setting will give you a lower AHI and be overall better for your therapy. As a pt you do have a choice. I am a big supporter of choices. I feel that a range of 6-15, 4-12, and even 9-15 is too big. Let's take Sherry's as an example 4-12. I have know idea what her optimal titration was, so for arguement sake let's say 11cmwp. She goes to sleep at 4cmwp. She either has to have an event or flow limitation to go to 5, then again for 6, yet again for 7, and 8, and then at 9 she seems to be titrated on her side. After an hour she rolls over on her back. Once again she has an event, flow limitation, and/or snoring the machine bumps her to 10, and then again to 11 before all events stop. The night might be half way over before Sherry reaches optimal titration. How much o2 did sherry lose to get to this point? In my opinion Sherry would have been better off if she had started at 11 in the first place. Even if Sherry prefers an auto setting if 11 is optimal the range woudl suit her better at 9/10-11/12.
Sherry was a bad example as she is on ASV. I assumed that she was on an auto set machine. sorry sherry this post does not apply to your treatment. I hope I still got my point across.
Hey, no problem, the above was very enlightening! I had no idea movements etc affected things that way.
Rock:

What do you suggest for straight cpap settings to begin with, I am tired of this back and forth nonsense with my doctor and supplier...... If I remember correctly you work in sleep a lab right??? I thought I was doing so well on straight cpap I just want to get on my machine to get better, would you mind giving me some clear suggestions what to start with....I am more than willing to go back to my supplier and get these settings changes back to straight cpap can you suggest what settings to start with.....last time when I was on straight cpap I was started at 4.0 ramping to 10 this took some time to adjust-- I think because the level needed to be higher--your expertise would be much appreciated if you don't mind......

I thank you ahead of time for any of your input Rock.....

Thanks

Belinda :)
