Q: "Do you have any personal experience with AutoPap? Also, since the machine is self-titrating, does it have to be pre-set to a specific pressure, or can one just start using it and let the machine figure it out?"

I am currently working with an APAP, and wondering if I should be satisfied with that or request to try a bipap. If the APAP jumps to a higher pressure due to a detected event, is the exhale pressure the same as the inhale pressure for that event response? Is there a bipap-apap machine or does it already work this way? My range setting is 5-12, with average pressure results around 7-7.5.

I find at times that the timing of the pressure increases can be disturbing when I am conscious and it is forcing me to take shorter shallower breaths than I want to which has me fighting the machine's flow pattern until it adapts to me or me to it. This takes some time and effort, delaying my full relaxation and time of falling asleep. It can be annoying at times, but usually does not prevent me from going to sleep eventually. I am not aware of waking up due to this effect, so maybe I should just accept it as is. I think this is more of a comfort issue than therapy adequacy issue.
You know, w/o knowing the exact brand and model APAP you are using it is difficult for us to make any real helpful suggestions for you.

Do you have "simple OSA" (by jnk's definition) or do you have any additional sleep disturbances such as PLMs, CSA, etc., etc.? Do you have any concurrent respiratory problems such as COPD, asthma, etc.?
Aw, come on, Rock Hinkle and jnk, I know you two are pulling each other's legs but .... we love and value you both and it hurts if some might get the impression you are yanking each other's chain.

I think of "simple OSA" as you do, jnk, but I can see where Rock Hinkle who conducts these PSGs night after night and constantly is exposed to the suffering that many go thru w/"only" OSA just can't accept "simple OSA" in the manner we do. Rock Hinkle CARES and seeing his patients night after night and what they go thru no OSA can be "simple" for him.
I winked and laughed when I said it! My meaning was that OSA had many faces. None of which are ever that simple. I have no hard feelings towards anyone. Especially jnk. I think he is sexy when he talks sleep. My response to the "secret handshake" was just a reminder. While there are no sleep police for any of you to deal with, for me it is different. The sleep police will, and have visited me regarding the post here on SG. I want to continue to help people at this site. It is just as much therapy for me as anyone else.
*giggle* Rock Hinkle, you are a hoot! I do have to agree w/you tho. I always cringe when I see certain information and URLs posted publicly in the forums. One of the things that makes me uncomfortable at cpaptalk despite how much I like that forum. There are some things that are just better shared "back channel", via PM or PRIVATE e-mail, when you feel comfortable the person is ready for such info. MOST of the time they are or they wouldnl't be in these forums, but every now and then ..... (consider yourself chastised, jnk! *wicked grin*)

Goshes, I really do HATE it when I disagree w/either of you two!!! Or w/RestedGal. *sigh* Just ruins my whole day.
Yer somethin' else, jnk. *giggle* But, you see, when I disagree w/the likes of you or Rock Hinkle or RG, it means I REALLY have to think over my thoughts on the issue. And I'm basically a lazy person and don't care to spend the time doing so. *wicked grin* When we disagree you guys make me think twice and that's a lotta work!!!
i understand that there is a formula based on this and that to get you started pre titration
second an apap can be used to titrate
a titration brings you very close to optimum
optimum can change under different conditions eg weight gain/loss
My understanding is that there is a pressure range programmed into the machine so it doesn't have to hunt through as many ranges available. What I found was when my auto pap was set at- can't remember exactly- say 20/14, it rarely went to the higher pressure even when I was having events.
