I believe a stretch injury to one or more nerves should have healed by now. So it sounds like he has either a partial injury or complete separation of one or more alveolar nerves. Bone graphs can potentially entrap a nerve and/or form a neuroma in a failed attempt to regrow. I doubt that there is a surgical solution to his problem, but I could be wrong. Meds designated for the treatment of neuritic pain should be at the top of the list for treatment.

Neurontin is one of those more commly used meds for neuropathic pain. It's an off lable use, but in some patients, it works pretty well from what I've read.

He is having sensory dysesthesias from nerve injury. I used to use Tegretol (an anti-seizure medication) for the treatment of this type of pain. It worked pretty well, but since I retired there are better medications, so I hear, for the treatment of neuritic pain. I'm surprised that the pain docs didn't find the right one.

Where exactly is the pain located? I assume it's in the region of the surgical separation of the mandible (jaw) or somewhere in the upper mouth (maxilla). At risk are the alveolar nerves. If one is injured or severed at surgery, a neuroma could conceivably develop, resulting in persistent pain. This is pure speculation, but I wonder if injection of a local anesthetic at the site would relieve the pain and if so, would resection of the presumed neuroma be the solution. I'm surprised that the pain specialists were not able to help him. Sounds like there were no ENTs or surgeons on board. The surgeon who performed the MMA should be able to refer him to the right specialist, since he has likely had to deal with this complication before.

I reviewed Dr Li's and Dr Powell's 400 case report of 2003 and they had 16 patients with persistent pain, but there were no details nor any comment on how these cases were resolved. Assuming Dr Li or Dr Powell were not his surgeons, you might consider making an appointment with one of them for an opinion and recommendations. My guess is that they have had enough experience with it to have come up with a solution by now.
