Hi John,
I actually work for Intermountain Homecare setting up CPAP and fitting masks. Normally you have a short list of providers to chose from depending on your insurance - you pretty much have to go with who are the preferred providers for your health insurance company or you end up paying a higher portion of the cost. I'd be happy to talk to you if you want to give me a call @ 716-5396.
