Hello,

  I am new to this board and just had a sleep study completed. I am a 42 year old female and have had insomnia for years.  I also have PVC's, which are premature ventricular contractions.  I take Toprol xl 50 mg.  I take this at night because it makes me sleepy.  I recently saw a neurologist who (along with my cardiologist) wanted me to do a sleep study.  The cardiologist wanted to rull out apnea as a possible cause of my PVC's and Neuro wants to see why I do not sleep well.  Neuro suggested melatonin 5mg at night.  I feel I am sleeping better with this addition.  I do unfortunately still wake up tired like I could just go right back to sleep, have headaches, memory issues, irritability, lack of concentration.  Here are the reults:

 

Sleep efficiency=85%

Sleep onset latency=34.2 mins

Stage REM latency=128 mins

Wake after sleep onset=30 mins.

Total sleep time=369 minutes

Number of awakenings=22

Stage 1=32 mins (8%)

Stage 2=239.5 mins (60%)

Stage 3=16 mins (4%)

REM 81 mins (20%)

 

SaO2 average awake=96%

lowest SaO2=93% (4 minutes)

 

Respiratory Events 

Apnea=0 events

Hypopnea-10 (3 rem and 7 non rem), index-1.6 per hour, average duration 31 and longest 153

 

AHI=sleep 1.6

RDI=1.6

 

HEART

Average heart rate during sleep=56

fastest heart rate during sleep=120

fastest heart rate during time in bed 197

 

Arousal summary

Respiratory index 1.3

leg movements .3

snoring0.0

spontaneous=23

Leg Movements=0

 

The recommnedation is that I do not warrant pap therapy, but possible treatments could include an anti snore device.  I am confused by this because there is no evidence of snoring according to the arousal summary for snoring.  They do suggesta MSLT, which I have done in the past.  It was inconclusive as I never fell asleep due to insonmia.  I feel the only reason I slept for this one is the Toprol makes me tired and the Melatonin helped!My questions are: If I do not have apnea, why am I so tired every day?  I always feel like I could go to sleep.  I have never woke up feeling refreshed, ever!  Why am I waking up so much?  What is causing this?    ANy suggestions, thoughts, etc are appreciated.

Now that your cardiologist ruled out sleep apnea, the  nocturnal tachycardia needs to be addressed. With that type of work-out, almost 200 while you're in bed, no wonder you're tired.
