I started cpap last april with ahi's of 34+. A couple of months later I was changed to a responics type 1 bi-pap auto. My ahi's slowly went down to 19 up until christmas. Now my ahi's have gone back up to 25+. My sleep nurse always tells me not to take any notice of my ahi numbers and it is how I feel that matters. I don't feel sleepy but I do feel permanently exhausted. I phoned her a couple of weeks ago and she just makes me cry. Now my machine has started to turn itself off and waking me up.My ramp starts at 15 and then auto from there. Idon't know what to do as I know I need to phone her. She used to be really nice but now I think she sees me as nagging, which, believe me I'm not.

Can you make an appointment to go in and explain that currently therapy is not working for you?  Don't phone- go in and see the doctor.  Make it clear when you make the appointment that you need to see the doc, not the nurse.

Make a brief but emphatic list of your symptoms.  An AHI of 25 or greater on treatment is not acceptable.  I'm afraid you may have to make the step of changing doctors if they cannot help you.  Exhaustion is a sign of under treatment. 

Call the DME about the machine, some of them have design flaws and turn off by themselves.  This equals broken.

Good Luck, hang in there.  I know this is a bad time.

So- call DME about machine.  Make an appintment with sleep doc.
