PR System One Auto CPAP - Philips Respironics' New Auto CPAP

Judy posted about the new PR System One Auto CPAP from Philips Respironics a few days ago, and I've been trying to dig up information to share with all of you since then. Today, I spoke to a very helpful and knowledgeable Respironics rep about the PR System One, and learned quite a bit about it. Here's what I found interesting:

- the machine's technology is sophisticated enough to detect when a central apnea is happening versus an obstructive apnea. it does this by building into its algorithm the delivery of a small "trial" burst of air down the airway and then measuring flow -- flow will be different in an open airway than in a closed airway. if there's an apnea in a closed airway, the machine knows its obstructive. if there's an apnea in an open airway, then the machine knows its central. Key point: this machine will not treat the centrals; it will merely identify and build the centrals and obstructive events into the data it collects and reports back

- the machine comes with a smart card (an SD card that is different from the standard Respironics smart card) that as far as I know is NOT compatible with the existing Encore Viewer software

- the DME can easily add a wired or wireless modem to the machine that will transmit the data to a clinician-friendly website (EncoreAnywhere website) and allow the clinical/medical team (sleep lab, physician, DME) to check compliance and efficacy data as well as make adjustments to the settings remotely, and to record their respective actions in a collaborative interface where everyone on the clinical team can see notes added to the patients record by any one clinician -- for instance if a DME's recent telephone conversation revealed something new about the patient, and the DME made a note of it, the sleep lab and physician would also be able to check in and read that new note

- the machine's built-in ambient room temperature and humidity detectors are designed to allow patients to use any humidification setting without experiencing "rain out"

- no improvements in efficacy data displayed to patient -- still patient only has ability to see 7-day and 30-day moving averages

- clinician can adjust settings through smart card, remotely via modem, or using "secret handshake" technique that is virtually identical to the "secret handshake" of the M Series boxes

- footprint larger than M Series

- it's at least as quiet as the ResMed S8, according to the rep. I believe the figure given was that it operated at the 20 decibel level, which is extremely quiet. He turned it on for me and admittedly, I could not hear a thing.

- that have a new "Dry Box" humidifier chamber that sits apart from, but which attaches to the side of, the flow generator. the separation between these two parts, and the design of the humidifier chamber, ensures that water cannot make its way into the machinery to gum up the works, as it often does when patients forget to remove the water from their humidifiers when they travel with the machine or bring it to their doctor's office.

- comes with a pretty and new carrying case

here's a shot of the front of the promotional material the rep was kind enough to give to me:

Nice basic information, but why would you say the rep was kind enough to give you that picture? Because that's not true. I personally cropped that photo your using. That is not the whole front of the brochure. I don't mind you ripping off my post from another forum, but please just give credit to where it came from. I broke the story of this machine (on another forum) and I worked hard to obtain the brochure (where this picture came from).
That said... by the way, I own this machine and can tell you more about it or answer any questions. Best regards!
geez... the rep was nice enough to give me a copy of the brochure. for the sake of speed, i grabbed a picture off the internet that had the image of the brochure already nicely cropped and in digital format. but here's a photo i just snapped now of the same exact thing from the physical copy that the rep gave me:

Does not help to defend yourself with her. It is what she does. I think it empowers her or something. Not enough time with daddy maybe? I don't know. She knows her sleep. I think she is a spy. Somewhere in Cuba. Intelligence ( AKA interrogation) Sleep is her cover. She has probably been past up for promotion by a few men.
No I am the model they used in the Picture and I've got the whole Ad layout right here in front of me. You all are wrong in jumping to conclusions like that. This Ad was not due to be released until 9/26/09 me and my agent will be looking for you SOCAL........You better cover your tracks.
So SoCal, how does it feel to use the PR One compared to whatever device you used to use (and can you tell us which one you switched over from)?

So far so good. I've only been on it one night. It certainly feels different with my same A-Flex settings (from old machine). I am not sure what it is... but it feels like the A-Flex response was some how different. I had a great first night on it. Very impressed. Absolutely much quieter than my old machine, which was an Auto-M with A-Flex.
I have an Auto M Series with A Flex too, and the noise from it never bothers me. whatever noise does hit me comes from my breathing in an out of the mask, which doesn't relate to the blower. did the noise of the M Series bother you? Or is it one of those things that you don't know how bad you had it until you have it really good?

If you would be kind SoCal to give us an update on your experience so far. I would appreciate it.
Yes, Mike - the noise from the Auto M w/ A-flex did bother me. Apparently I am a light sleeper. I am now sleeping through the whole night. How much of it is the noise from the new comfort/therapy from the PR? I don't know. It has only been 5 days for me on the new machine.

What I can say so far, is that I really love it. Aside from the software issue (which PR says they are working on a patient version)... all is going well. Encore Pro 2.1 is out and does work with it.

A couple of things I have discovered.

1. The new SD card mounts to your PC (or Mac) desktop, without any special drivers. You can even read a couple of the text files there. Of course, nothing with important data. It will copy the data from your machine to any SD card without any special formatting. Any SD card with a Fat 32 format works. So backing up your data is easy.

2. It has a GMT Offset adjustment. That means you dial in (with it's control wheel) the offset for the time zone your in area. Here on the west coast, -7 (for daylight savings time) allows the machine to not only sync with the correct time for the software... but it allows you to get your data from the LCD screen on a noon to noon basis. That means at 12:00 noon, I can see the 7 day & 30 day averages from the previous night. The old M Series wouldn't do that. The data on the LCD would change at 5 AM (in my time zone), so you would never know exactly what data you were looking at.

While the machine is not perfect. In my opinion it's a huge improvement over the old series which is being discontinued. When I can access the data again (one way or the other) hint hint... I will then be 100% happy. But for now... this is the future of the PR line. The old series is history.

In person, it is a beautiful machine. Much nicer looking than the picture indicate. Also the light grey area around the control wheel, lights up from the back with numbers (you didn't know were there) in an arc around the wheel, showing you the humidifier number setting. They fixed the "blue night lights" (that many of us had to cover). The LCD can be back lighted or not. Other lights go off after a brief period of access. So your room can remain completely dark.

The new humidifier is fantastic! You can see the water level. Easily remove the tank. Add water and slide it back in. If you have to remove the tank lid for cleaning. There is a release clip in the center that pops with your index finger. Then easily snaps back into place, I used to hate the old M Series humidifier and stopped using it. This one has the capacity to go the distance. And not rain out problems yet with in monitoring of not only the temperature but the humidity of the room as well. And the hose placement rotates. I've never had it get hung up on anything. The new performance hose is very lightweight and matches the light grey theme of the machine. It looks cool connected up.

The sound level is much lower than the "less than 30 db" spec they provide. At least in the Auto A-Flex mode... I never hear it. Just a little noise from my Mirage Activa LT Nasal mask... it rocks! I love it... a very very silent mask. So I am in silence heaven.

Overall, I love the machine. It's very well built and with a great job of writing the firmware interface. It's cool new technology. What more could ask for? Software! That's all. Oh, and maybe they could do a better job with the LCD data reporting. But I can live with it until software. But it's all easy to access and you can maneuver round the menus... so easily that to anyone can it figure without referring to the manual. They did a really good job on the whole device.

Can you tell I like it?
Great, detailed review. You're the only one I know who actually has the machine. Good to hear that you're still loving it despite the patient software snag. I wasn't aware that there's a new "performance hose" out. Whenever I order new supplies, the hose seems to be an afterthought, and they throw in something cheap looking and generic that isn't even made by the manufacturer of the machine.

I'm totally new to the boards. I just got my first CPAP today, and it's this machine. Your detailed review here makes me pretty happy. I'll try to remember to come back and talk about my experiences with it. :)

Brian, you're well on your way with the most cutting edge CPAP technology out there. I wish I had gotten off to such a good start ;-)

Please do let us know how you like it.
