Hello friends at Sleep Guide Discussion Board. I just got back from an appointment with the Pulmonary/Sleep Specialist and was told the following.

My sleep study from last Tuesday evening was very interesting. Based on the result my AHI score were 106.5 per hour and this was one of the worst cases of central sleep apnea she had seen. They believe that my CSA is in direct correlation with the brain tumor removal I had in April 2007 in addition to oxygen saturation levels dropping down to 70% at times throughout the night. Their recommendation is a follow up study with the iintroduction of CPAP useage and the they want to try to find the appropriate mask. They are recommending the ResMed Liberty mask since I already know I can't use the Ultra Mirage full face mask due to seal problems in the past. They have also indicated to me that the equipment they would like to use in this situation is the Adaptive Servo Ventilation (ASV) but insurance requires proof of failure using CPAP before they will authorize ASV useeage/purchase. Is this a common practice for insurance to require use of CPAP, which is least expensive treatment, before they authorize the ASV technology? And has anyone out there on the discussion board had experience using ASV?

It is very common practice for insurance companies to require failure of CPAP before paying for ASV. Some companies will also require a failure of BPAP on top of the previously failed CPAP before they reimburse for ASV.

I am no expert with the ASV units, but as a tech I do have some experience with them.
Rock is right. Some insurance co's need the failed CPAP before a VPAP/ASV test can be done. At the sleep lab I work in some cases central sleep apnea or complex sleep apnea must be diagnosed during the First sleep over (if you will). We have been having a lot of cross chat about central sleep apnea, and I want others to know that this is one of those rare cases I have been going on and on about. Larry the ASV should work to lower that 106AHI, What it does , unlike CPAP is motivates you to breathe when your brain forgets to and when you start breathing again it adjust so the shock doesn't wake you. UPDATE: I used my CPAP last night for most of the night (I woke at 12 and was awake for 30 minutes). It is amazing how well I feel this morning after one night! As the Doctor told me yesterday this is something that I must do, unless I want to take a chance of not waking up one day. The technician who worked with me said that the hardest part of CPAP compliance is finding the right mask.Hopefully with a committment on both our parts, we will be able to find the correct mask best suited for me, Has anyone out there used the ResMed Liberty Mask? That is the what they fitted me with yesterday and I love the fact that it is open around my eyes and I can still wear my glasses to read before falling asleep.
