I received the doctor's dictated report on both of my studies -- the sleep study and the titration study. Our local hospital outsources their record keeping and I have been unsuccessful in obtaining the full scored report.
I have tried numerous times and have sent the HIPPA paperwork and still all I have received is the dictated reports. I don't actually see the sleep doctor that read the tests and he is associated with our local hospital. When I contacted him he stated that the reports are the hospital's responsiblity and they are the ones I need to contact. When I contact the hospital I am told no reports are actually "kept in hospital" but are done through a company in Georgia. I was instructed by the hospital to go through this place in Georgia and all though I have talked to some RTs about what I need to ask for the dictated reports are all that is sent (I have tried 5 or 6 times). I don't think this company knows their rear from a hole in the head as they haven't figured out what I am asking for. At this time I have given up on getting them.
I would point out that these full scored data summary reports w/condensed graphs and the equipment order (prescription) are part of our medical records and in the USA we have a legal right to copies of them under HIPAA. (The originals are the property of the generating facility or doctor).
While it isn't always in our best interest to alienate a stubborn doctor by forcing the issue as far as filing a complaint w/the Department of Health & Human Services under HIPAA and with our state licensing boards and accrediting societies there are times when we don't intend to stay w/that doctor and it would be worth it. Usually we can get around such doctors by having our family or other doctor request the full information and provide us w/the copies.
But for those who DO want to push the issue to the limit I'll have to post the link to the HIPPA form letter and guidelines.
I call it the full scored data summary report w/condensed graphs. The sleep labs so far have gotten the drift of what I mean. They are usually some 5+ pages for each study.
