Seems to be a special process for getting forehead pad replacements for CPAP masks -- i ordered mine the other day and the DME seemed to be caught off guard by the request. Am I the only one to have ever ordered a replacement for this piece?

Mike, so far I've replaced cushions and whole masks, but never had the need to replace, or thought of, replacing the forehead
pad. I can imagine (strictly imagination, but I'll ask my DME next time I see him) that it might be an unusual request. The DME says he basically has two kinds of clients- zealots like those of us here that keep our equipment in top shape and use it, and those who for one reason or another don't tolerate CPAP and aren't interested in help to do so.
Good for you for keeping the mask in top condition.

I think they're usually taken aback because people usually just replace the entire mask since insurances typically allow for replacement after 3-4 months.

Just a thought, I don't know for sure why.
after a few months our clients get a new mask. You are allowed replacements every few months.
