I flew to Hawaii, not International, but didn't have any trouble with my bag not being treated as carry on and not counted as carry on. I ended up checking my bag on the trip home- too much to carry and after I almost fell down the up escalator (bad Balance) I decided CPAP and maybe a backpack were enough to carry.

Thanks for the information. I returned from my dive trip and had no problems traveling with the cpap machine. I did do what Mary did on a trip and checked the unit on the way back instead of carrying it.
