Changing machine settings for ramp up on respironics m auto c flex

OK. I am only entering day two of Auto CPAP usage and am already wanting to change settings :-)

The DME set my ramp up time for 45 minutes per my instructions. I thought that in the beginning, I might have trouble getting to sleep, would be easier with lower pressure etc...thinking that I could just adjust the ramp time myself later. I tried to take a nap this afternoon and realized that it was silly to have it ramping up for half the time I was trying to sleep so wanted to adjust it to 20 min or so.

I have the respironics auto M cflex machine. Instruction book says I have to get the DME to reset the ramp time. Is that really true? YIkes!

Is there a workaround for that. Why would they put something like that "behind a locked door"?

Ramp time on an Auto is not really necessary since the pressure will stay low while you are awake and only adjust up as you need it. The problem with delaying the time (in particular the Respironics M-series) is that by the time it wakes up to measure the air flow you might already be in the later stages of sleep and several apnea episodes weere missed. THe algorythm Respironics uses does what's called a "hunt and peck" method. The machine actually stops measuring the airway vibrations, resistance and absence of flow for 5 minutes at a time. When it does "wake up" to take a look it will adjust it's pressure by 1.5 cmH2O increments until normal flow is re-established. Then it goes back to sleep and stays at that higher pressure until it wakes up again 5 minutes later. This cause over compensation at times and will cause sleep disturbances due to too much pressure.

By leaving the ramp feature off the machine is more dynamic and will make adjustments more gradually as you fall asleep and your airway begins to relax so by the time you hit that stage of REM were your pressure requirements are at their highest you have already slowly built up to those pressures and the chances of sudden pressure changes causing distirbances is lessened.

And BTW. No there really isn't a way for a non-clinician to make changes to your APAP settings on the Respironics devices. The Resmed devices do allow the user to change ramp time. That is why I only dispense the Resmed product.
In addition to my previous comment. The ResMed Autoset II & Vantage use an algorythm continuosly sampling the last five breaths make adjustments in 0.5 cmH2O increments to till changes in flow and resistance is over come by 95%. The usual result I have found comparing the two products is the end result in effective pressures are signifacantly lower with the ResMed than Respironics. About 1-2 cmH2O lower. It may not seem like much but when it comes to making mask choices and finding that perfect one that, not only is comfortable but seals, 1-2 cmH2O can make or break it.
