Advice - comparison of machines - Remstar Auto M vs. Resmed Autoset II

Hello,

I'm frustrated as hell, as usual, about my interaction with the wonderful world of sleep apnea therapy and I could really use some advice.

My DME just replaced a defective Remstar M Series with A-Flex with a Resmed Autoset II.

I would like some feedback about these two machines, i.e. if they are comparable to each other.

I was expecting the Remstar to be replaced with the same model, but the tech told me that the DME was out of stock of the Remstar and that the Resmed machine was comparable because it was also auto-adjusting.

I wasn't at all happy about this switch, because I've adjusted quite well to the Remstar, whereas my first machine was a Resmed Escape II CPAP machine and I couldn't stand that machine. But the tech assured me that this Remed Autoset II machine was completely different from the Resmed CPAP.

This is my first night with this Resmed machine, and I'm awake after 2 hours, throwing the mask off of my face because the pressure from the mask feels so uncomfortable.

Is this Resmed Autoset II machine truly an equivalent replacement for the Remstar Auto M A-flex?

Press and hold the Left and Right arrow buttons for 4-5 seconds. This gives quick access to the Quickview menu Settings section (for patients to view their device settings in read-only mode) and the Results section only. Review should appear.

Press the Left arrow. Settings should appear.

Press the Down arrow button.

Now tell us what each setting is, use the Down button to proceed to the next screen until the last screen has an UP arrow instead of a Down arrow under the printed screen information. Then press the Right arrow key to Exit each screen back to original Ramp or Settling screen.

So you are able to GET to sleep but can not STAY asleep? Knowing that's a big help. It tells us that the Ramp or Settling are set at a good time period for you. Whether it is set to Ramp or to Settling will tell us if your AutoSet is set in straight CPAP mode or in Auto mode.

Settings:

EFFICACY DATA
PRESS: 15.0 day...
LEAK: 2.30L/s day...
AHI: 6.7 day....
AI: 0.4 day...
HI: 6.3 day...

Actually, I consider the AutoSet II the better device. But to each his own.

Since the device shows Settling it is set in Auto mode. Were you using the Respironics in Auto mode?

What mask are you using? W/o the correct Mask Selection setting the data may not be reliable. I would assume the Mask Selection most likely is correct but that is also assuming that your provider knows what he/she was doing which all too often is NOT the case.

What were your pressure settings on the Respironics? Are they set the same on this AutoSet II?

Your leak rate is WAY TOO HIGH. We'd prefer to see it not higher than 0.4 L/s (24 L/M). And beyond 40 L/M the device has problems compensating for the Leak and the data is most likely skewed.

Your AI is considered good at 0.4, "we" like to see it no higher than 1.0. Your AHI isn't all that bad considering the Resmeds do tend to score hypopneas more aggressively than other brands.

Your reported pressure of 15 cms is considerably higher than the 8 cms which is most likely caused by the device trying to compensate for that excessive Leak.

Back to your data again. How long have you been using this AutoSet? The AutoSet will report 1 week's average data as well as the previous night's data.

What couldn't you stand about the Escape II? The Escape II is a straight CPAP. You went from it to an APAP so its not all that surprising that you preferred the APAP. Don't let your dislike for that straight CPAP compliance data only Escape color your appreciation of the fully data capable auto PAP AutoSet II. Keep an open mind.

I'm pretty sure that the Respironics machine was in Auto mode, it was adjusting to my in and out breathing.

The mask is a Fisher and Paykel full face mask.

The tech just copied the setting from the Respironics machine to the Autoset II.

I have been using this Autoset II for about 2 weeks now.

With the Escape II I couldn't adjust to the constant pressure. I would either knock the mask off in my sleep, or half wake up and remove the mask. I adjusted almost immediately to the auto PAP Remstar.

I seem to be adjusting to the Autoset II now also. It almost seems as if it started out with full constant pressure and that it's been learning and adjusting to my breathing since then.
