I just had my data read soooooooooo disappointed My AHI is 14.8 Hypopnea Index is 11.1 Apnea Index is 3.8 this is all bad....

The data showed leakage--my husband watched me sleep early this morning--I use the Mirage Swift II with a chin strap--I thought I was still opening my mouth but, my husband said that my lips are being blown open by air....I'm not sleeping with my mouth open!!! Is this normal???? What does this mean??? Should I be changing masks--I really like this mask it took me a while but, I finally found one.....

I am using the Resmed Auto-set S8 with air settings settling from 5 to 12 over 25 minutes if anyone out there can shine some light on what is happening I would appreciate the feedback --I see my sleep doc on Monday....good grief I really thought I was doing so good, because I was feeling better....

HELP!!!!!!!!!!!!! :( :(

The fact that you are feeling better says that you are doing well. Do not get discouraged. Bumps are going to happen. your AI is low. It will not take very much to reduce your HI.

It is the nature of auto devices to allow you to have events. I highly recommend that you ask your doctor to switch you to a straight pressure. Just my opinion.
Hi Belinda:

"Remain calm. All is well!"

The other Rock is right: you feel good, your AI is under control, & things are looking up. My personal view is that the breakthrough exhalation through the lips is not a big deal since apneas & hypopneas are problems in INhalation, not EXhalation. The trick is to establish whether the breakthrough is inhalatory or exhalatory.

Since you really like that mask, hang with it.

Above all, relax. You are better, & there is more improvement in store.

Rock
Thank you so much the both of you.....I agree that I probably need to to be switched back to straight cpap......

I feel much better with your reply....am feeling more calm

Thank you sooooooooooooo much guys....
Jik:

I am off to Wal Mart to pick up that stretch headband--I hope this works better than the chin strap I have.....I just want the leakage to stop--if I feel an improvement with leakage I can just imagine without it YAHOO!!!!!! I am very anxious to get a handle on the leakage problem :)

Meegwetch (thank you) guys
Ach, Belinda! Your AI (which is THE important one) is 3.8 so you are doing okay, how you are feeling isn't lying to you at all. Always believe your body rather than some machine, even a good Resmed!!!

I experience "the lip flutters" too. I haven't found tho that my lips stay "open" after a lip flutter. I've told the story here some time previous of waking myself up one night w/a loud whistle like some men use to call a dog. It came out of the corner of my mouth. Scared the heck outta me and took me a while to realize what it was.

5 to 12 is a pretty wide spread or range of pressure. You "might" want to ask your doctor or DME supplier's RRT about trying moving the bottom setting of 5 cms up to 6 cms for a week or two of data. If that shows an improvement in your AI you might even consider trying 2 weeks at a pressure range of 7 to 12 cms. Once your AI is consistently below 1.0 I'd be satisfied - assuming I was still feeling good and rested, of course.
Judy:

I thank you very much......glad to know that these "lip flutters" are normal for us SA people lol--I will definitely ask my doc to increase the air levels...what chin strap do you use Judy--I remember some time ago you had posted it somewhere but cannot remember what discussion you shared that with the group....

Meegwetch (thank you) Judy :)
The lip flutters are why you see so many people advocating mouth taping. The chin strap I use has reduced my Leak rate quite dramatically - but - it has not totally stopped the lip flutters. No chin strap can do that. It takes either mouth taping to prevent the lip flutters or a full face mask to render the lip flutters totally immaterial to our Leak rate.

However, other than a home-made panty hose leg, etc. the Respironics basic white chin strap PN 302175 is the cheapest chin strap on the market and it is comfortable and working well for me. My lip flutters are on exhalation to the best of my knowledge. The only way to know if they are also occuring during inhalation would be to ask the sleep tech to pay particular attention during an in-lab titration.

Rock Hinkle and Rock Connor know better than I. Rather than the pressure changes I suggested I'd listen to them and try what they suggest instead.
Thank you Judy:

I am thankful for your quick response......I tried the headband wasn't successfull :( but, not giving up on this I am ordering the same chin strap as you....hopefully this will work better for me......I really like my mask I am using that was one of the biggest battles.....so much wisdom here.
You need to get that gap reduced 5-12 is too large it gives the machine to large of an area to mess up. A difference of 3 to 4 insted of 7 would lower your index 2 fold, You give the machine an inch it will take a mile. I dont even know if you were titrated or not, if you were just given an auto-PAP then you are fighting an uphill battle.Try setting it to 7-12 for a few nights, and see what happens? 5-12 wont solve anything. Good Luck.
Thanks Duane:

All this feedback is very helpful.....my supplier of course doesn't say much about anything thank goodness for this site and people here with knowledge I will set if for 7 tonight.....

thanks again :)
Judy:

Is it safe to tape your lips shut to stop the lip flutters???? Or do you leave a small hole to allow some air to escape????

Thanks:)
Belinda did you have a titration study?

An auto-adjusting machine is designed/programmed to keep your pressure at the lowest it can. these machines have to have events to work. you have a few events the pressure goes up. This happens until the pressure is raised to a point that all events stop, or you have reached your setting limit (12cm for you). this is where I see the problem with auto-adjusting machines. After a certain lenght of time without events the machine will lower the pressure, and continue lowering until you begin to have events again. An auto-adjusting pressure will allow you to have more events than a straight cpap set to your optimal pressure.

it is my opinion that an auto-setting is not good for long term use. Others may disagree, but i will argue with them. An auto pap is designed to keep your pressure as low as possible while keeping your events at a minimum. It should be the other way around. there is no such thing as a good event.
