Anyone know how one lands in a Medicare HMO vs. regular Medicare program? Also, at a high level, what's the difference between the two?

To the best of my knowledge, you have a choice of doctors, and if you choose to use an HMO doctor, then you are in an HMO that accepts Medicare. If you do NOT want to put up w/an HMO DON'T chose an HMO doctor! You can go to ANY doctor you choose as long as that doctor accepts Medicare patients. From time to time some doctors will limit the number of Medicare patients they will accept until their practice balances out and then will begin accept new Medicare payments again. I've never know of a doctor to release patients who are on Medicare, just to limit any NEW Medicare patients.
