I woke up about 5 am this morning with no mask on. Not only that, I had opened all the straps and taken off the nasal pillows. I have no remembrance of doing this or no idea when I did it. I decided just to turn over and go back to sleep....and discovered even this little bit of not using the cpap was not good for me.

Just wondered how common it is to "tear" the mask off and toss it during the night
"unconsciously."

In the sleep lab this is very common when the proper amount of pressure isn't being delivered.

Reasons why this isn't happening:

Large mask leak
Large mouth leak
Hole in tubing (unlikely)
CPAP machine isn't set to the proper pressure (optimal pressure not found)

Another reason could be is that your mask simply isn't comfortable.

If you're new to CPAP it could be that you aren't used to having something strapped onto your face.

If you wake with a dry mouth it could be a mask leak. Try heated humidity (or increasing), try a chinstrap, try a full face mask, use saline spray to clear nasal congestion prior to going to bed.

Issues common to using nasal pillows:

Direct nares insert often causes a greater likelihood of nasal dryness that leads to nasal inflaming, that leads to you not getting enough air and.....off goes the mask.

Nasal pillow masks usually go to claustrophobic patients and are designed to feel like they're aren't strapped to the head so tight. Result: They aren't strapped to the head so tight! :^) A mask with a more robust headgear is much tougher to get off without being conscious of it.

This has only happened twice to me. The first time I got up to go to the bathroom and remember going back to bed and it felt so good when I laid back down I promised myself I'd put my mask on "in a minute" but woke the next morning w/no mask on. The second time I have absolutely no memory of taking the mask off but am willing to bet the same thing occurred.

Altho not at all "claustrophobic" I am one who much prefers "the less, the better" on my face. I've found that I love the Respironics Simplicity simple nasal cushion due to its being so lightweight and fitting lower than the bridge of the nose. I rather like the Resmed Swift LT for similar reasons. I'm not quite so fond of the nasal pillows as I am the Simplicity simple nasal cushion tho. I also rather liked the OptiLife but found the chin support a bit of a bother as its not meant to be a chin strap to keep our mouth closed and I couldn't draw it up tight enough to work as one yet it interfered w/my using my chin strap.

Even in chin straps I find I prefer the Respironics Basic White chin strap, PN 302175, since it consists only of a silky, soft chin "saddle" and one simple strap up each side of the head attaching at the top of the head.

I've also found hanging the hose above my head reduces the "drag" of the hose on my mask as I move from back to side or side to side which can disrupt our sleep.
I still toss my mask during the night from time to time. I have 6 different mask at bedside. prefer full face mask. I am going to try and review the newest total face mask that cover entire face. I am very closterphobic, can't promise a great review.
Conn I think you are the first sleep vet. that I have heard of that prefers a full face.
