Last Friday I had the appointment with my sleep doc and told him I was still feeling tired. He ordered a pressure increase by two points. Today an Apria tech came out and adjusted my pressures but before leaving she made sure that they were right. She hooked up her machine and got a confused look on her face. She told me that at an 8 setting it was reading at 5. It all makes sense now why I've been sleeping so crappy. My machine is broken :(. Thank god it was figured out when it was so I can start sleeping better. She left the new pressures on the machine and she's going to call ResMed about getting me another unit in a couple of days. I still have the old one to use until then. I really thought that there was something wrong with me. I kept thinking "Why do I still feel like crap?" I thought I had some other medical issues besides the apnea and being overweight. Now I know that that's probably not the case. If I would've changed the pressures on my own like my sleep doc told me to do I would have never known the machine was broken. When I first got the machine I would wake up rested and ready to go. I wonder if it broke just recently? Hm guess there's no way to tell....

Anna, I wouldn't worry about when it broke, it's just good news that you found the problem and hopefully you can get back on track quickly.

I think I would ask for a replacement now.  It is not your fault it is broken, and the company must have machines in stock! Just does not seem right you should have sub optimal treatment for any more days!  Hope it works out quickly for you.

Anna, I agree with Ginny especially with Easter coming up it may take longer. Even if they can give you a different machine till they get yours in stock.

The tech called me back today after she left. She told me that there's feature on the ResMed S9 Elite that when you exhale the air pressure goes down then back up when you breath. She said that when this feature is on it can give a false reading. I'm going in tomorrow to have her turn off the feature and check it. If this is the case that the pressures go up and down and my machine is okay I'm going to see if it helps tonight with the new pressure.

Right, Expiratory Pressure Relief (EPR), a comfort measure.

The EPR Effect will be clearly seen once you get your software:

where Therapy Pressure is the Set Pressure and Average Pressure is the Observed Pressure (in this case, EPR is set at 2.0).

Aanna, the ERP feature is important, make sure they get it working, you can change it anytime you want like the humidity. Mine was originally on set at 2 and I found it reasonably comfortable, I miss read the instructions and turned it to 0 and could breathe, so I I cahnged it to 5 and have felt very comfortable since, but it is a personal preference.

What do mean by "get it working"? If EPR is set to 3, obviously it's already working.

What needs to be looked at is AHI and Detailed Graphs.

Do I understand this correctly?  If my pressure should be set at 10 and the EPR is 3, the effective pressure could be 7 .  Therefore if I raise the pressure to 13 I would get a effective rate of 10.

I want the answer too!   I know I would not be happy without the EPR feature on.  Mine is at the highest setting.  Accidently one time the RT at my DME company turned it off.  Most uncomfortable to me.  I played around and figured out how to turn back on myself.

The EPR feature is expiratory pressure  relief.  It reduces the pressure by whatever it is set to (1,2,3, cm) on exhalation only.  Some people need it and find it comfortable.  I do not use it (it is called flex on my respironics machine).

To explain a bit further if your IPAP or prescription is for 10 and you use EPR you will still be getting 10 when you inhale, depending on where you set your EPR your exhalation pressure would be that much less.

Mary's correct, the EPR doesn't have anything to do with your pressure setting. It is just for your comfort on exhalation similar to the humidifier setting can be changed for personal preference. I found the highest number best for me and it is a matter of trial and error because a setting for one person is not the same as a setting for another person.
