Narcolepsy and hypersomnia are intrinsic dyssomnias that cause excessive sleepiness.

4-Narcolepsy- Is a disorder characterized by excessive sleepiness with abnormal REM sleep, cataplexy(sudden muscle weakness), hypnogogic hallucinations(usually at sleep onset), and problems sleeping at night. A person with narcolepsy will have sleep onset REM(SOREM) followed by short inconsistant burst of REM. A narcoleptic's sleep cycle does not usually follow what we would consider a normal pattern. It is estimated that 3 million people worldwide are aflicted with this disorder. Of that 25% have been diagnosed.

5-Recurrent hypersomnia- People with this disorder have recurrent episodes of extreme sleepiness and huge sleep needs. Episodes usually last several days to weeks, and occur at least 2x a year, on average(They can occur at least once a month). Pts will sleep as much as 18-20 hours a day, waking only to eat and use the bathroom. The best described cause of this is Klein-Levin Syndrome which mostly afflict teenage boys. Pts with this syndrome will sleep long hours and binge eat when they are awake. They also might be hypersexual.

6-Idiopathic hypersomnia is a disorder in which the pt complains of excessive sleepiness and prolonged sleep at night. The sign that sets people with this disorder apart from the normal long sleeper and narcoleptics is that there are numerous episodes of NREM sleep that last for up to 2 hours. For this reason, this disorder is sometimes called NREM narcolepsy.

7-Posttraumatic hypersomnia is excessive sleepiness that results from an physical injury, or disease to the central nervous system. Usually this will go away if or when a pt heals.

As extreme sleepiness and a large sleep need(debt) can be a symptom of many other disorders, like narcolepsy and Apnea it is important to rule these out with a PSG. Following a PSG a Multiple Sleep Latency Test(MSLT) is performed to evalute Excessive Daytime Sleepiness(EDS).

An MSLT will began within 1.5 to 3 hours after the PSG.

I am sure that some of my pts have one of these disorders I have yet to perform an MSLT personally.

I believe that is it for Intrinsic Dyssomnias and disorders 1-12. I hope you are enjoying this. in trying to find information on sleep disorders I have found the number of sleep disorders vary from person to person and book to book. I have heard numbers anywhere between 70 and 111. In thruth I do not know where the sleep disorder count will end. If I missed any please add them.
