I had a sleep study on Wednesday, the 8th.   It was a trial of ASV for me.  I haven't been able to achieve an AHI even close to normal range and I have lingering, drug resistant depression, plus I'm on a laundry list of meds, some quite sedating so my doc ordered a trial of ASV.  We started out with straight CPAP and at some point switched over to ASV.  It was one of those nights I just did not, or at least felt like I didn't sleep.   Part of that was once the ASV kicked in the pressure was all over the place and I was having lots of trouble with mask leaks at the higher pressures.  I understand now how people wonder "did I sleep enough for the study?".

I have had the same tech for 4 sleep studies so I know him very well so I asked him the night before that if (in hindsight this makes no sense because of the nature of ASV)  he thought I would be better off at another pressure than my CPAP 9.4  would he think about telling me.  He said that if he did my doc could never know.  But the next morning he said the ASV was all over the place so there was no one pressure he could recommend.

Even though I didn't sleep well that night I had the greatest sense of well being the next day that I have ever had on the machine.  For the first time since starting CPAP  I actually felt like a normal, non depressed person.  Life was good.  This was on one night's sleep, not even good sleep, with proper breathing.

I'll see the doc as soon as I can get an appointment to go over the results, but I sure hope I can at least do a longer trial of the ASV.  It was very difficult to get used to- I never did in one night.  I've been having insomnia and the changing pressure was distracting because of the mask leaks it caused.  But, I felt so good the next day!

I'll post the results of the test when I see the doc.

Hi Mary, Did they say what your o2 sats were? Good Sleep,Chris

The first appointment I could get with the doc was March 1st to go over the results.  My transportation problems are the hold up to getting in quicker.  I have never desaturated in all my sleep studies and 2 overnight pulse ox tests, so it will be interesting to see if anything has changed.  The tech at this lab is an excellent tech, but a real by the rules kind of guy so the most I could get out of him was "the ASV was all over the place".  Thanks for asking.  As soon as I can after March 1 I'll post the results..

I've been using an ASV for 3 nights now.  It is wonderful!  Still having some problems with my nasal pillows but it seems so effortless, so much easier than the bipap st I was on.  I think I must have a deviated septum, because even with the Lanisoh, it feels like there is a hole being burned in the inside of my nose.  I'm actually dreaming again with the ASV, I wasn't getting any REM sleep before.  I'll be interested in what they tell you about your sleep test.  Good Luck!

My doctor called me last night and told me the ASV had worked for me and he would be calling in a prescription next week. Glad to hear you are doing well with the ASV, Nancy.

That's great news Mary, and I hope this will means better therapy and health/well being for you :)  Will keep an eye on this post.

Well, after 18 days my AHI is down to 8.9 from 28.  I'll see the doc is another couple of weeks.

I can't get the sd card to download data so I haven't gotten a breakdown on AI and HI.

That's good to hear Mary - sounds promising.  Do you feel any better with a lower AHI?

Kath, truthfully I can't tell any difference.

Aw, that's a shame.  Maybe you will with time (fingers crossed for you).

Mary Z, those figures are fantastic for you. Remember what we tell others that it may take time. I know it's been a long time for you but stay positive that it will happen and hopefully you will be able to lower your meds.

Well after   30 days my AHI has risen to a 30 day average of 10.9, but I got the software- haven't really studied the data yet, but I have days with an AHI under five.  I don't sleep 10-12 hours a night any more, don't get sleepy as early in the evening, have completely stopped Provigil, and my lingering drug resistant depression seems to be lifting.  I figure it will take 3-6 months to feel the full benefit of the ASV.  I'll see the doctor soon to see if any adjustments need to be made.  I have an EPAP of 8, a max IPAP of 20 and usually run a pressure support of 5.3, so I guess that means my usual IPAP is 13.3.  My Tidal volume is more consistent at an average of 300.  I don't know why I couldn't achieve the same results in any other mode of therapy.  This is expensive, but after just 30 days I have to say it's worth the money (my last machine was paid for- this one is a more expensive rental).

This is sounding brilliant Mary and your patience at reaching this place is paying off.  Hoping for more positive news as time goes on :)
