I had a sleep study on Wednesday, the 8th.   It was a trial of ASV for me.  I haven't been able to achieve an AHI even close to normal range and I have lingering, drug resistant depression, plus I'm on a laundry list of meds, some quite sedating so my doc ordered a trial of ASV.  We started out with straight CPAP and at some point switched over to ASV.  It was one of those nights I just did not, or at least felt like I didn't sleep.   Part of that was once the ASV kicked in the pressure was all over the place and I was having lots of trouble with mask leaks at the higher pressures.  I understand now how people wonder "did I sleep enough for the study?".

I have had the same tech for 4 sleep studies so I know him very well so I asked him the night before that if (in hindsight this makes no sense because of the nature of ASV)  he thought I would be better off at another pressure than my CPAP 9.4  would he think about telling me.  He said that if he did my doc could never know.  But the next morning he said the ASV was all over the place so there was no one pressure he could recommend.

Even though I didn't sleep well that night I had the greatest sense of well being the next day that I have ever had on the machine.  For the first time since starting CPAP  I actually felt like a normal, non depressed person.  Life was good.  This was on one night's sleep, not even good sleep, with proper breathing.

I'll see the doc as soon as I can get an appointment to go over the results, but I sure hope I can at least do a longer trial of the ASV.  It was very difficult to get used to- I never did in one night.  I've been having insomnia and the changing pressure was distracting because of the mask leaks it caused.  But, I felt so good the next day!

I'll post the results of the test when I see the doc.

Hi Mary , Have you ever had your vitamine D3 levels checked? Dr Stasha Gominak a Neurologist talks about vitamin D on utube in a 5 part series. It  has me curious , this could potentially be helpfull. She said a level of 60 to 80 is needed for optimal sleep.Good Sleep,Chris

Well, I have settled in with the ASV at have an AHI average of 8 (used to be 28 pre ASV). For the last two months I have only wanted to lay on the couch.  I walk my dog and then want to lay on the couch the rest of the day.  Of course this is not possible- I have housework and yardwork to do.  I had a physical and saw the sleep doc and nothing is out of whack there.  My only thought now is this must be depression slipping back in.   I have gone back to taking my four hour naps.  My psychiatrist increased my antidepressant.   One other thought is I am on heavy opiate pain relievers and this is the cause of my depression and lethargy.

Hi Mary, Sorry to hear the asv is just part of the solution . I Had blood work up and vascular study only to find that my lethargy may be my bp med . Sunshine and fresh air help boost vit D and helps some. I guess thats why its a" medical practice" not medical doing. Good Sleep,Chris

Mary, you know 8 is good compared to 28, if it gets below 5 you are normal, I know that doesn't help when you still feel tired etc. The antidepressants and other meds you are on are probabaly causing it but you have to take them if they help the depression, see how upping the antidepressants go and hang in there.

I am stunned about how much your AHI has come down.  You surely deserve it after years of trying different machine settings and soooo many masks as well as having to find ways for the mask to even possible fit in some fashion.  But, you never gave up on it.  

Now, I am sorry to hear of your wearing tiredness.  Most people don’t understand how exhausting it is to be sooo very tired and incapacitated which, of course, leads to frustration and disappointment, if not, outright discouragement.  I am very sorry for your increased lethargy and tiredness.  

I have kept myself together over these past 4-1/2 years of having to sleep too much at night and to sleep during the day as well and, therefore, losing so much time in days, months and years, I think, by believing the reasoning that that I can gain a renewed zest and capacity for activity through investigation, persistence, tenacity, meditation and trying to be patient and kind with myself throughout,.  I mention that because you have exhibited all of those measures, AND MORE, yourself.  You have been dealing with not only with sleep problems and tremendous fatigue but with many other inhibiting illnesses as well.  I, sincerely, hope for you that you will have some respite soon and much more to follow.

Well, I seemed to have settled on an AHI of between 8-10 which is  much better than my 28 on my S8 Vpap Auto 25.  All in all I'm pleased, adjusted easily to the machine- don't even feel the pressure anymore.

I saw the sleep doctor a couple of weeks ago and my AHI is now four to eight from their previous record of 18.6.  I had a couple of med changes from my psychiatrist and my depression has passed.

Great to hear Mary, and just shows too how the depression is worse with a higher AHI.  So glad you're feeling better for the holidays :)

That is great news!!!  Even the spending $$ part!  Glad you are finally getting to feel better.
