After 5 years of CPAP, my current doctor's hypothesis is that reliance on CPAP has weakened the muscles in the back of my throat, and that I should work with a speech pathologist to strengthen those weakened muscles. I also took a home test and my AHI was down to around 5, which, if the home test is accurate, might be related to having lost about 30 pounds since my initial study, which had me at an AHI of ~ 18.

My doctor's thinking is that to the extent the speech pathologist can strengthen the muscles in the back of my throat, I might be "cured" and no longer need CPAP. Not sure i'm convinced -- i.e., I'm not convinced -- but this is certainly a new twist in my journey with Sleep Apnea that I thought I'd let you all know about. Will keep you posted.
Ask Dr. Park: Cutting Edge Surgical Options for Sleep Apneatag:www.sleepguide.com,2012-02-18:2549090:Topic:2074402012-02-18T06:09:27.004ZSteven Y.
Discovered this site after a Google search for "cpap+tongue" because a side effect of "fat tongue" recently developed. The effects are quite alarming and I plan to see both my pcp & sleep doc asap! I feel like a speech impediment has suddenly developed. Initially, I felt as if my upper palate had stretched and shifted my teeth! But, after reading some of these articles, it's quite possible "weak muscle tone" is the symptom. Thank you for this Forum!!! Janis

Also, I was perscripbed an oxygen concentrator with the nasal noduals. I, for the most part get about 5-6 hrs of sleep at night.

I went to the local VA medical center and saw an Ear, Nose and Throat specialist and he told me that my tongue was too big, I never realized it before, until he said something, and now I look at it and go WOW that thing is huge.

I went to him because I had read that a deviated septum could be the cause of my sleep apnea, well he said I don't have one, and he said that there have numerous surgeries to fix the size of my tonugue.

I guess my big question is, what is entailed in a surgery of this sort, and most important is this procedure very painful?

I am getting to a point where I am noticing the oxygen on every night, and of course this keeps me awake.

During sleep most muscles relax. This is true of the tongue muscles as well. By providing gentle stimulation to the hypoglossal nerve, normal daytime tongue muscle tone is restored to key muscles and the tongue does not fall into the throat, the airway is kept open and the patient can once again breathe normally during sleep.

An incision is made in the upper neck. The hypoglossal nerve is readily identified medial to the digastric muscle and the electrode is placed around the nerve. The electrode is connected to a small pulse generator (IPG) implanted in the upper chest wall. Potential implantation complications are few, primarily infection, estimated to be 1%.

It stimulates the hypoglossal nerve and returns daytime muscle tone to the sleeping tongue and prevents it from falling back in the throat and obstructing the upper airway. The implant is recharged through the skin once or twice a week. It is turned on by the patient with a handheld radio-frequency device prior to sleeping. It then runs continuously throughout sleep, and is turned off in the morning. Not requiring additional sensors or synchronization. The THN Sleep Therapy stimulator can be reprogrammed by the physician as often as required.

It is the physicians responsibility to determine those patients whose obstruction is tongue based.
